Wellness, Prevention, and Disease Screening: This plan is for patients who are concerned about their future health and are interested in assessing if latent chronic disease states are present in their body. In this plan, the patient has access to our Boston clinic for:

• An annual visit that includes a comprehensive health screening, health assessment, blood tests, educational program, health report card, and a personalized health maintenance plan.
• Follow-up visit that occurs within the first year to measure patient progress compared to the base-line health assessment established during the first visit.
• Five (5) hours of phone consultation directly with the patient and/or with the patient primary care physician.
• If the health assessment reveals latent disease, the patient is encourage to enroll in the Disease Management program. The cost of this program will be reduced by the payment already made for the Wellness, Prevention, and Disease Screening.
